Parent Permision:
All parties who hold Parental Responsibility need to complete the Parent Permission form Social workers may also complete this from. Please send us an email if you have any queries about permission as there are of course some special circumstances where it may not be possible to gain consent.
School Referral:
All referrals need a referral form from either the parent or school, depending on who is referring for support. SDQs from both Parent/Carers and School are essential for all school referrals. Parents who self-refer can choose whether to involve the school in the referral process.
