All parties who hold Parental Responsibility need to complete the Parent Permission form (under certain circumstances, one parent is possible). Please send us an email if you have any queries about permission.
All referrals need a referral form from either the parent or school, depending on who is referring for support. SDQs from both Parent/Carers and School are essential for all school referrals. Parents who self-refer can choose whether to involve the school in the referral process.
